To use our app, ask your cardiologist or cardiac rehab clinician for a remote delivery option on referral or during your first visit to the cardiac rehab clinic. Your cardiac rehab clinician will provide login details, show you how to use the app and help you set goals for your rehab. The app will help you track your blood pressure, weight, symptoms and exercise, plus provide you with educational videos and keep you motivated with in-app messaging. Your clinician will review your progress and provide the support you need to recover during weekly phone calls.
